Nurse Practitioners
Heather Champagne, PNP
Heather F. Champagne joins the Pediatric Group of Acadiana as Board Certified Pediatric Nurse Practitioner. She received her Associates Degree in Nursing from Franciscan Missionaries of Our Lady University in 2009 and her Bachelors Degree in Nursing from the University of Louisiana Lafayette in 2014. She furthered her education and received a Master’s Degree in Nursing from the University of Alabama Birmingham in 2016. Before joining the Pediatric Group, she served as a nurse in the Pediatric Emergency Room at Our Lady of the Lake Children’s Hospital for nine years. Heather is also a Pediatric Clinical Instructor for Franciscan Missionaries of Our Lady University and she is on the Standard Setting Committee for the Pediatric Nursing Certification Board. Heather lives in Greenwell Springs, Louisiana with her family and she is excited to bring exceptional pediatric care to the Maurepas area.
Jamie St. Romain
Jamie Bayham St.Romain is a Family Nurse Practitioner who comes to us with 3 years of experience in family practice, specifically rural health. Her background includes primary care of patients of all ages (from newborn to geriatric) as well as women’s wellness health and contraceptive management. She earned her Bachelors Degree in Nursing from Southern University in Baton Rouge, LA in 2010. She then practiced as an operating room Registered Nurse at Our Lady of the Lake in Baton Rouge for 4 years. She returned to graduate school in 2013, and in 2015 she earned a Masters in Nursing as a Family Nurse Practitioner from UL Lafayette. She is AANP board certified. Jamie is a resident of Pointe Coupee Parish and is excited to be coming home to serve her very own community! She is married to Cade St.Romain and together they have 2 children, a three-year-old daughter and a one-year-old son.
Courtney Habetz, PA
Courtney Vice Habetz, PA-C received her undergraduate degree in Biology from the University of Louisiana at Lafayette in 2004. She then completed the Physician Assistant program at Louisiana State University Health Science Center in Shreveport, Louisiana in 2006. Courtney is very happy to return home and provide adolescent and women’s health care to south Louisiana. She is married to B.J. Habetz, and they have one son, Tucker and two daughters, Emmy and Blakely.
Hollan Fortier
Hollan Deshotel Fortier who joins our practice as a NP-C received her Bachelor’s Degree as a Registered Nurse from Northwestern State University in 2007. Hollan then furthered her education completing the Advanced Practice Masters program from the University of Louisiana at Lafayette graduating as a Board Certified Family Nurse Practitioner in 2013. Hollan has experience in the medical surgical unit, post anesthesia care unit, and cardiology specialty. Hollan is married to Karl Fortier III and they have two daughters Ava-Grace and Vivian.
Lisa Alexander
Lisa Alexander joined our practice as a Board Certified Family Nurse Practitioner. She received her Bachelors Degee in Nursing from McNeese State University in 1998. In 2013 she furthered her education and received a Master's of Nursing Science from Northwestern State University and became certified by the American Academy of Nurse Practitioners. Before joining Pediatric Group she practiced in the Emergency Department at Lafayette General Medical Center for 15 years as a nurse and nurse practitioner. She was born and raised in Abbeville, LA, and now lives in Lafayette with her family and practices in our Church Point office.
Alison Cantrall, NP
​Alison Cantrall joined our St Martinville Adult Internal Medical practice as a Board Certified Family Nurse Practitioner. She received a Master’s of Nursing Science from the University of Louisiana, Lafayette in 2013. Throughout her advanced practice education she earned the honors of Summa Cum Laude and was selected by faculty as the outstanding graduate. Prior to that, she received her Bachelors of Nursing Science from the University of Louisiana, Lafayette in 2009, as well as a Bachelor of Science in Marketing and Economics from ULL in 1999. Alison brings with her a passion for health and wellness, with experience in medical surgical units, intensive care units, wound care, hematology/ oncology, palliative and end of life care, as well as plastics and reconstructive surgery. Alison is married to Alexander Cantrall and the mother of four children Jonah, Juliana, Jillian Paige, and James.
Diane Kramer Gordon, PNP
Diane Kramer Gordon joins our practice as a Board Certified Pediatric Nurse Practitioner. Diane received her Master’s Degree in Pediatric Advanced Practice Nursing from Old Dominion University in Virginia in 1998. Diane has experience in critical care and emergency nursing and brings 20 years experience as a primary care provider to the pediatric population. Diane was born and raised in Baton Rouge and now lives in Lafayette. She will be practicing in our Crowley office.
Katherine Delahoussaye, APRN-CNP
Katherine Delahoussaye was born, raised, and currently still living in New Iberia, Louisiana. She graduated from Westgate High School. She then attended the University of Louisiana at Lafayette, where she received her degree in Health Promotion and Wellness in 2013, as well as Nursing in 2017. She then went on to McNeese State University where she completed the Family Nurse Practitioner program in December of 2022. Before becoming a Nurse Practitioner, Katherine practiced in the Emergency Department at Iberia Medical Center for 9 years. Katherine is the mother of Asher and twins Kaylei and Khylon.
Kristyn Wunder FNP-BC
Kristyn Wunder is a dual certified nurse practitioner coming to PGOA from Lafayette, Louisiana and will be practicing as an Internal Medicine nurse practitioner in Lafayette, LA at the Womens and Children’s location. She earned her Masters in Nursing at the University of South Alabama as both a Family Nurse Practitioner and an Adult Gerontological Acute Care Nurse Practitioner in 2017. Prior to her receiving her Master’s in Nursing, Kristyn earned her Bachelors in Nursing at the University of Louisiana at Lafayette in 2011. Her work background includes working at Ochsners Lafayette General Medical Center for almost 2 years on the telemetry floor and years in intensive care unit as a RN. She then went on to practice as a nurse practitioner and has been practicing now for almost 6 years. As a nurse practitioner she has experience practicing in cardiology, urgent care, and family practice. She has a passion for health and wellness, and preventative care. Kristyn is very excited to be a part of PGOA. She is married to Jared Wunder and together they have two daughters named Rosemarie Grace and Eden Faith.
Stephanie Zeno FNP
Stephanie Zeno is a Family Nurse Practitioner who currently works at the Broussard Pediatric Clinic in Lafayette, LA. She has one year of experience as a Nurse Practitioner including Pediatrics and Urgent Care. Stephanie earned her BSN in 2016 and MSN in 2022 from THE Southern University of Baton Rouge, LA. Stephanie's prior background as a RN includes Pediatrics, MedSurg, Psych, and Geriatrics. After spending over fourteen years at Our Lady of the Lake Children's Hospital, she then started her traveling career in 2018. She now resides in Breaux Bridge, LA engaged with three beautiful children Jarrriona, Jarriel, and Philip Jr. Her role as a NP is to help integrate health maintenance, disease prevention, health education and treatment of common problems in primary care and pediatric settings. Stephanie looks forward to providing exceptional, passionate care and support to both the patients and their families here in Lafayette and surrounding areas.
Crystal Broussard MSN, APRN, FNP-C
Crystal Broussard MSN, APRN, FNP-C is originally from Glencoe, La. She currently resides in Lafayette Parish. Crystal attended Delgado Charity School of Nursing and the University of Louisiana at Lafayette (ULL) where she earned her undergraduate degrees in Nursing. She later attended Simmons University in Boston, MA where she obtained her Master’s of Science in Nursing-Family Nurse Practitioner degree in January 2017. During her time in Undergraduate & Graduate school she was a member of the Sigma Theta Tau International Nursing Honors Society. Some of Crystal’s other accomplishments include: Best Clinical Student Award (Delgado Community College Charity School of Nursing) 2008 & 2009, Who's Who Among Junior College Students 2009, First Church of God in Christ Living Legend 2014 honoree, & First Church of God in Christ 2019 Influential Pace Setter. Crystal is a current member and is certified by the American Academy of Nurse Practitioners (AANP). She has practiced nursing for 19 plus years and has been a Nurse Practitioner for almost 7 years. During her 19 years of practice she has worked in Med-Surg, Rehab, LTAC, SNF, Retail health, orthopedics, and primary care. Crystal states, “My focus in healthcare is to contribute to the path of better health for all patients focusing on wellness, prevention, maintenance, and patient education by integrating clinical practice, evidence-based research, and education, to provide the best quality care within my scope of practice that is safe, effective, efficient, and patient focused. My goal is for all patients to live a longer and healthier lifestyle. I am excited to be back in Acadiana to provide my services to the community and I look forward to building a great relationship with all my patients.” Crystal is the mother of 3 boys Michael, Jackilon, and Lenox.
Crystal Hancox MSN, APRN, FNP-C
Crystal Hancox MSN, APRN, FNP-C is from Youngsville, LA and currently lives in the Lafayette area. Crystal attended the University of Louisiana at Lafayette (ULL) where she earned her undergraduate degree in Nursing in 2004. She returned to the University of Louisiana at Lafayette in 2013, where she obtained her Master's of Science in Nursing-Family Nurse Practitioner degree in December of 2015. Crystal is a current member and is certified by the American Academy of Nurse Practitioners (AANP). She has practiced nursing for 12 plus years, as well as being a Nurse Practitioner for 8 years. Her nursing experience include Pediatric ICU, Pediatric ER and ER. She brings with her experience in both Primary Care and Urgent Care as a Nurse Practitioner. Crystal is very excited to join PGoA at our North Lafayette clinic and looks forward to providing services and building relationships with the community in North Lafayette. She is married to her husband Jeremy Hancox, and together they have two amazing children Emery and Daxton. In her spare time, Crystal enjoys being outdoors, spending time with her family and friends and watching her children play sports.
Laksamana Nasantara, APRN-CNP
Laksamana Nusantara (Sam) is committed to delivering exceptional patient care across the lifespan. He obtained his Bachelor of Science in Nursing at the University of Louisiana at Lafayette in 2016, followed by extensive experience in an inpatient setting, specializing in Medical-Surgical and critical care environments. In 2023, he completed his Nurse Practitioner education at McNeese State University, showcasing his dedication to advancing his clinical knowledge and skills. Immediately after, he was hired by Becton Dickinson Company as a Clinical Educator in Fort Lauderdale, FL where he excelled in educating healthcare professionals and enhancing clinical practices. Despite his professional success with BD, Lafayette has always held a special place in his heart. This year, he decided to return to his alma-mater hometown to join PGOA. His commitment to lifelong learning is further demonstrated by his pursuit of a PhD at West Virginia University. With a strong background in both clinical practice and education, Laksamana Nusantara brings a wealth of knowledge and a compassionate approach to his patients. He is dedicated to improving patient outcomes and advancing the field of healthcare through his work at PGOA in the Crowley clinic.
Ross Dugas, APRN-CNP
Ross is originally from the St. Martinville/Catahoula area. He graduated from Our Lady of the Lake College in Baton Rouge in 2009 with an Associate of Science in Nursing. In 2019, he obtained his Bachelor of Science in Nursing at the University of Louisiana at Lafayette. In May of 2022 he graduated from Nicholls State University with a Master of Science in Nursing, Family Nurse Practitioner concentration. He is an ANCC Board Certified Family Nurse Practitioner and Louisiana State Board of Nursing Certified Family Nurse Practitioner.
With 15 years of nursing experience, Ross has cared for all ages in numerous acute care settings including the emergency department, medical and surgical departments serving as both staff nurse and charge nurse, the critical care unit, and case management. He also spent two years further expanding his knowledge base in cardiology within a local group. Most recently he practiced in a clinic where he truly realized his calling in family care.
When not working, he enjoys spending time with his beautiful wife, Linzy, and three kids: Khori, Beckett, and Saylor. He is thrilled to have the opportunity to bring his vast experience and dedication to provide compassionate and comprehensive healthcare to the community of Henderson/Cecilia and surrounding areas.
LaDaja Allen, MSPAS, PA-C
LaDaja Allen, MSPAS, PA-C is a certified Physician Assistant from New Iberia, LA, who currently resides in Lafayette, LA. LaDaja earned her Bachelor’s degree from Lane College in Jackson, TN, majoring in Biology with a minor in Chemistry. During her time at Lane College, she conducted extensive research, presenting her findings at prestigious conferences, including the American Society for Biochemistry and Molecular Biology in San Diego, CA, and the American Society of Cell Biology in Washington, DC. She is also a proud member of Delta Sigma Theta Sorority, Inc.
LaDaja completed her Master of Science in Physician Assistant Studies at Christian Brothers University in Memphis, TN, and is an active member of the American Academy of Physician Assistants.
LaDaja is passionate about serving her community and working in rural health. She is dedicated to providing high-quality, patient-centered care. She says, “I am excited to use my skills and training to make a meaningful impact in the lives of others, especially in underserved areas where care is needed most.”
In her free time, LaDaja enjoys spending time with her family, friends, and her beloved rescue dog.
